Compensation: $18.00 - $22.00 / hour

Description

We are currently seeking a highly motivated Spanish-speaking Care Coordinator for our Care at Home Program, which providers primary care services to patients in their homes! This role will operate on a hybrid schedule out of our corporate office and will report to a Care at Home Program Supervisor to provide direct operational and care coordination support to a clinical team delivering in-home care to high-risk members.

The Care Coordinator plays a key role in supporting members with complex medical needs following hospital discharge or identification through internal referrals. The position focuses on coordinating services, managing referrals, obtaining medical records, and serving as a central point of contact between members and external partners to support care stabilization and prevent avoidable readmissions.

What You'll Do
  • Responsible for member and provider outreach (telephonic, written, and electronic) related to the Home Care Program, utilizing department templates and workflows
  • Coordinate scheduling of in-home visits and follow-up appointments for the clinical team
  • Assist the clinical team with non-clinical care coordination activities, including but not limited to:
  • Obtaining medical records from various healthcare facilities
  • Generating, submitting, and tracking referrals and authorizations for services ordered by the clinical team
  • Coordinating durable medical equipment (DME), home health services, labs, and other ancillary services
  • Monitor and manage incoming Home Care Program communications
  • Track and document member outreach attempts, missed calls, and text messages in designated systems
  • Answer inbound phone calls from members and assist with needs or appropriately route concerns to the clinical team
  • Provide feedback and input on care coordination processes, workflows, and tools to help improve program efficiency and member experience
  • Adapt to workflow changes and new responsibilities as the Home Care Program continues to expand
  • Participate in health plan, department, and program-specific meetings and trainings as applicable
  • Meet standard benchmarks with performance objectives
  • Assist in delegated projects and assignments related to the Home Care Program
  • Perform other duties, projects, and actions as assigned
Qualifications
  • High school diploma OR GED
  • Fluent Spanish
  • Healthcare experience in medical assisting, referral coordination, care coordination, scheduling, and medical records
  • Experience using Microsoft applications such as Word, Excel and Outlook
  • Ability to manage multiple priorities in a fast-paced, high-acuity environment

Environmental Job Requirements and Working Conditions
  • Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. Hybrid work arrangements are contingent upon role requirements, employee performance, and business needs and may be subject to change. The office is located in Houston, Texas.
  • The national target pay range for this role is $18.00 - $22.00 per hour. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors.
